Fill in this form if you want to set up a business account with us. This will allow you to place orders via text, email, phone or visiting our showroom. Also, you can request a dedicated sales person to visit your establishment.

2. New York State Resale Certificate (Optional)


If you intend to purchase items to resell without paying sales tax, please fill in this form.
